[PVE-User] Migration VM from VMWare 5 (.vmdk) to Proxmox

I need to migrate a VMware ESXi 5 _LINUX guest_.
In the VMware environment I have 6 files:
     SRV-flat.vmdk
     SRV.nvram
     SRV.vmdk
     SRV.vmsd
     SRV.vmx
     SRV.vmxf

Can you show the right procedure to do the migration ?
