[PVE-User] io-error

I have had vms go in to a state where the status is "io-error", and they
never seem to recover. The vms in this state seem to drop off the network.
The vms don't respond to shutdown, so I have to stop the vm. The "io-error"
status also often persists across vm stop/starts.
My vms are all running ubuntu 12.04 lts.
Does anyone have any idea of what this is and/or how to stop it from
happening?
This is very annoying as ubuntu 12.04 vms that are not shutdown correctly
(just stopped) end up taking forever to come back online, this might be due
to some kind of ext4fs repairing.
