I see this happen now and then but it has never caused a crash.
Experienced this with:
- Debian Wheezy and Ubuntu server 10.04 and 12.04
- FreeBSD 8.x - 10.x
- Omnios 151008 - 151012
- CentOS 5,6,7 and RHEL 5,6,7
