Have you tested with open-iscsi running? The problem is that insserv 
does not know that qemu-server depends on iscsi.

When i add iscsi to qemu-server init script:

# Required-Start:    iscsi pve-cluster cman clvm $network 
$local_fs$remote_fs
# Required-Stop:     iscsi pve-cluster cman clvm $network $local_fs 
$remote_fs

everything works fine and as expected.
