I was a problem with my iscsi config, imported from my old lenny.
clean iscsi config works fine.
By the way, do you have bugtracker? I don't wan't to harass you with mails ;)

seem to come from
iscsiadm --mode discovery --type sendtargets --portal 10.5.0.18
iscsiadm: discovery login to 10.5.0.18 rejected: initiator error (02/0b), non-retryable, giving up
iscsiadm: Could not perform SendTargets discovery.
so maybe this an wrong open-iscsi conf somewhere in /etc/iscsi ...
I'll try manually.

I'm trying to add an iscsi storage.
I add my portal ip (10.5.0.18), then click on the target combo.
I have an error 500 in firebug

i think is related to /usr/bin/iscsiadm , but i can't find errors logs.

it was my fault,sorry, i was doing building on an old server with apache2 with mpm worker already installed.
i'll try to install all the .deb on a clean squeeze install, to test if it's working fine.

How did you install the package? We depend on apache-mpm-worker (see debian/control.in)
